Activities

Handcrafted with Love and seasonal blooms, this posy box is a delightful introduction into the art of Floral Arrangement. This rounded design will brighten up your coffee table or kitchen bench where everybody can see and smell its delightful scent.

This minimalistic design uses groupings of blooms, free flowing use of space, harmony and balance throughout the arrangement. The contemporary style particularly invites the viewer’s eyes to observe the arrangement from the front. The creator will arrange the flowers to display balance of the mind, provide timeless architecture of space and elegant ambience.

This arrangement is a selection of seasonal wildflowers native to Australia and South Africa, presented in a beautifully designed gift box. These long-lasting flowers may be dried for an everlasting arrangement for the home.

This floral arrangement uses a beautiful mix of seasonal cut flowers designed for placement into your own vase. Express yourself and your creativity whilst arranging these flowers into the perfect hand tied bouquet.

Create a petite wedding flower posy for the Bride to be using seasonal cut flowers bound with satin ribbon and pearl headed pins. Enjoy making the mother of the Bride a delicate corsage that she can wear proudly on her wrist.

Use all the skills from the past 5 weeks to create your very own Designer’s Choice arrangement. The base of this arrangement will be wrapped in hessian fabric and will lend itself to either the contemporary or posy style arrangement, or a combination of both with the help of your support team.

There are facilities available for free tea and coffee. A fridge and bench space is available for prepare and store any food or drinks participant may bring.

Location: Helensvale Community Centre- 31-37 Discovery Drive, Helensvale

– This centre is wheelchair friendly with ramps and large disability toilet

– Support is provided at a 1:4 ratio. You are welcome to bring your own support worker at your own cost and our team will support both the client and Support Worker. If you require 1:1 support, we are happy to provide that as well

– Tea and coffee making facilities are provided. Please bring water bottles or any food items you may require during the 3 hour workshop

– This is a 6/8 week program. If the participant chooses to disengage before completion of the program, 2 weeks notice must be given and will be charged accordingly.

 

Goals

 

– These workshops support participants to build capacity and skills for daily life. They give participants the opportunity to build relationships, forge friendships and increase their social support.

– Workshops improve cognitive functioning including your ability to plan, organise and problem solve. They also improve your abstract thinking and creativity.

– The items create and add value to your living environment and often require ongoing care and nurturing.

– These skills assist to achieve the goals you set in your NDIS plan.

 

Timetable

The NDIS and this service agreement
This Service Agreement is made for the purpose of providing support under the participant’s National Disability Insurance Scheme (NDIS) plan The parties agree that this service agreement is made in the context of the NDIS, which is a scheme that aims to:


– Support the independence and social and economic participation of people with disability, and
– Enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports

How will payments be made?
Lotus Disability Care Group Pty Ltd  will Claim the agreed cost of the workshop attended from the NDIS, through participants elected plan management company.

Cost of Service?
See attached a copy of the Program of Support.

 


Responsibilities of the Participant


The participant/participant’s representative agrees to:


– Inform the provider about how they would like the team to deliver support at the workshop
– Treat the provider with courtesy and respect
– Talk to the provider if the participant has any concerns about the supports being provided
– Extend courtesy and notify  the provider if they are unable to attend a workshop
– Give the provider the required notice if the participant needs to end the service agreement and let the provider know immediately if the participant’s NDIS plan is suspended or replaced by a new NDIS plan or the participant stops being a participant in the NDIS.

What is expected of the service provider?


– Communicate openly and honestly in a timely manner
– Treat the participant with courtesy and respect
– Support the participant to access and manage in workshops consistent to their needs
– Consult the participant on how the workshops will be managed
– Give the participant information about managing any complaints or disagreements and details of the provider’s cancellation policy (if relevant)
– Listen to the participant’s feedback and resolve problems quickly
– Protect the participant’s private and confidential information
– Provide supports in a manner consistent with all relevant laws, including the National Disability Insurance – Scheme Act 2013 and rules, and the Australian Consumer Law: keep accurate records of the supports provided to the participant

 

Terms and Conditions of the Service Agreement

 

Ending the Service Agreement

Should either party wish to end this agreement, we must give 2 (two) weeks’ notice. If there are serious breaches to this agreement, the requirement of notice will be waived. If there are further queries or questions regarding this, please email us at info@lotusdisabilitycare.com.au.

What to do if there is a problem
If the participant wishes to give the provider feedback, is not happy and/or wants to make a complaint, the participant can talk to:

The parties agree to the terms and conditions of this Service Agreement; and the Program of Support.
